A case review found that four-year-old Daniel Pelka, who was murdered by his mother and her partner, was described as being “invisible” to teachers, social workers and police and that “no professional tried sufficiently hard enough” to talk to him.

Daniel was beaten and starved for months by Magdelena Luczak, 27 and Marius Krezolek, 34 before his death in March 2012.

The pair were jailed for 30 years each after being found guilty of his murder in August this year.

Birmingham Crown Court heard how the four-year-old had seen a doctor about a broken arm and had arrived at school with bruises and facial injuries. The trial heard from a teaching assistant who described Daniel as a “bag of bones” and the court heard how he was “wasting away” and often scavenged for food in bins.

At the time of his death, Daniel weighed just over a stone and a half. The report did not blame a specific agency but stated that “critical lessons” had to be “translated into action”.
